Government-Issued ID Documents
(Donor + Witness)

To register for whole body donation, we need a clear copy of a legal, government-issued ID (front and back) for both the prospective donor and the witness

This helps us confirm the identity of all parties involved, ensures your registration documents are completed correctly, and allows us to accurately record and honour your donation wishes.

Accepted Identification Documents

As part of our whole body donation programme, we accept a range of valid government-issued photo identification documents. This allows us to confirm the identity of both the donor and witness, helping ensure your registration is completed correctly and your donation wishes are honoured.
