To register for whole body donation, we need a clear copy of a legal, government-issued ID (front and back) for both the prospective donor and the witness

1. Donor Details
As part of our whole body donation programme, we accept a range of valid government-issued photo identification documents. This allows us to confirm the identity of both the donor and witness, helping ensure your registration is completed correctly and your donation wishes are honoured.

A valid UK passport can be used as accepted government-issued photo ID. Please provide a clear copy of the front and back.

A current UK photocard driving licence is accepted as valid government-issued ID. Please submit a clear copy of both sides.

A non-UK passport with a biometric chip is accepted as valid photo ID. Please provide a clear front and back copy.

A valid UK Biometric Residence Permit is accepted as government-issued photo ID. Please provide a clear copy of the front and back.

We accept valid government-issued photo identification such as a passport or driving licence for both the donor and the witness. This is required to confirm identity and ensure the registration process is completed correctly.

No. Please do not send original documents. We only require clear copies or photographs of your identification. Original passports or ID documents should never be posted to us.

Passport: Please provide a clear copy of the open double-page spread of the passport (not the front and back).
Driving Licence or other photo ID: Provide a clear copy of the front of the card.

Yes. You can submit your registration forms along with clear copies of your identification digitally, or send them via email or post.

If you do not have either form of photo ID, you may submit:
A copy of your birth certificate,
A copy of your NHS medication list that includes a photo of you
These documents will be used to help verify your identity for registration purposes.

We do not accept non-government issued identification such as:
Bus passes
Travel cards
Student ID cards
Library cards
Store or membership cards
These cannot be used as proof of identity for registration.
