To complete your whole body donation registration with A Love For Humanity, you will need to fill out four essential forms and provide valid
government-issued identification.

These documents ensure that your donation wishes are clearly recorded, legally verified, and can be honoured at the appropriate time. Each form plays an important role in confirming your intent, identifying key contacts, and validating the registration through an independent witness.

1. Donor Details

This section collects your essential personal, medical, and contact information required to create and maintain your donor registration.

  • Provide your full name, address, postcode, and date of birth to confirm your identity for registration.

  • Include your telephone number and email address so we can contact you regarding your donor registration if required.

  • Indicate any relevant medical information, such as the presence of a pacemaker or implanted medical devices.

  • Provide the name, address, and contact details of your registered GP, along with your NHS number.

  • Record your marital status and legal next of kin details to ensure appropriate contact can be made when needed.

2. Donor Wishes & Preferences

This section allows you to outline your preferences for how your donation may be used for education, training, or research purposes. It also records your wishes regarding retention duration, final arrangements, next of kin notifications,
and related consent options.

  • Select how long your body may be retained for education, training, or research purposes.

  • Indicate whether you consent to the respectful public display of your body or body parts for educational use.

  • Confirm if you allow images or recordings to be taken for education, training, or research, where you will not be identifiable.

  • Choose your preferred option for the final respectful disposition of your remains following donation.

  • State whether your next of kin may be informed of relevant test results or receive updates about donation-related research.

  • Indicate if you are currently receiving palliative care or have a terminal diagnosis to support timely registration.

  • Confirm whether you are currently registered on the organ donor register.

As part of our whole body donation programme, we accept a range of valid government-issued photo identification documents. This allows us to confirm the identity of both the donor and witness, helping ensure your registration is completed correctly and your donation wishes are honoured.

Accepted Identification Documents

  • A valid UK passport can be used as accepted government-issued photo ID. Please provide a clear copy of the front and back.

  • A current UK photo card driving licence is accepted as valid government-issued ID. Please submit a clear copy of both sides.

  • A non-UK passport with a biometric chip is accepted as valid photo ID. Please provide a clear front and back copy.

  • A valid UK Biometric Residence Permit is accepted as government-issued photo ID. Please provide a clear copy of the front and back.

  • A valid UK Biometric Residence Card is accepted as government-issued photo ID. Please provide a clear copy of the front and back.

How to Provide Your ID Copy

To help us verify your identity securely and process your application without delay, please follow the guidance below when submitting your identification documents. Providing a clear and complete copy ensures we can review your information quickly and avoid the need for resubmission.

  • For Passports: Please provide a clear copy of the open double-page spread of the passport

  • For all other ID types: Please provide a clear copy of the front and back

  • Clear and readable (no glare, no blur, or cut-off corners)

  • Shows your full name, photo, and document details

  • Submitted as a scan or high-quality photo (PDF or JPG / PNG)
